Long Term Conditions
The Care Services Improvement Partnership is working to encourage development and improvement in community and vocational rehabilitation in London so that people with long-term neurological conditions (LTNC) are enabled to lead a full life in the community. The intention is to ensure that:
People withLTNC living at home are to have ongoing access to a comprehensive range of rehabilitation, advice and support to meet their continuing and changing needs, increase their independence and autonomy and help them to live as they wish. (Quality Requirement 5 of NSF for Long Term Neurological Conditions); andand
People with LTNC are to have access to appropriate vocational assessment, rehabilitation and ongoing support to enable them to find, regain or remain in work and access other occupational and educational opportunities.(Quality Requirement 6 of NSF for Long Term Neurological Conditions).
Conference - Quality Commissioning – a fuller life for Londoners

This conference focused on good practice in London, focussing in particular on commissioning and developing ways to enable people with long-term neurological conditions (LTNC) to lead fuller lives and be active participants both in their communities and in the workplace.
